Disaster Preparation and Response

Preparation:

Residence Permission Form for Disaster

This form allows residents of Light and Life Park to authorize volunteers or park personnel to take specific actions in their homes during a disaster or emergency situation. These actions may include opening windows or doors and turning off electrical breakers to ensure safety and prevent damage.

By signing the form, residents acknowledge and accept the risks associated with these services, which are provided free of charge by the ministry. The form includes a waiver of liability, releasing Light and Life Park and its volunteers from any claims or damages that may arise during the provision of these services.

The agreement also includes a clause for binding Christian arbitration in the event of any disputes, and affirms that the resident has read and understood the terms of the release.

Please download this document. Fill in the blank fields, and either email it back to the Light and Life Park Office or mail it to the office via the United States Postal Service.

Please click the button to the right.

Click on the button below to print the form and fill it out by hand.

Then you can do one of the following:

  • Send it by United States Postal Service to the office at 5604 Deeson Rd., Lakeland, FL 33810

The volunteer group, Inspiritus, has a grant to help people who are still rebuilding from Hurricane Milton. They were here in the park last fall/winter and performed volunteer work. They are looking for people to help again this fall and winter. If you are interested in using their volunteer services, their flier is available for download to the right. The flier says residents of Polk County but their grant also includes Hillsborough County. Also, this does not need to be your primary residence. So, you don’t need to be a year round resident of the park to qualify. Just use the QR Codes shown above to contact this organization or call (404) 875-0201 Extension 1023.
